Medical Clearance Process for Student Athletes:
- Enroll in the KNICA 199 out-of-season conditioning class for your specific sport .
- Print and complete this Medical History form.
- Print and fill in your name and birthdate on this Physical Clearance Form.
- You will take both forms with you to your primary care physician (or another sports medicine physician) to be completed. The physical form must be completed and signed by an MD (medical doctor), DO (Doctor of Osteopathy), FNP/NP (Family Nurse Practitioner) or PA-C (Physician Assistant). The physical form will also need to be stamped by the office to verify that you were there. We are not allowed to accept a physical performed/signed by a physical therapist or chiropractor.
- The Medical History form will not be signed by the physician but will just help the physician do a more thorough physical and will need to be returned to us along with the Physical Clearance Form.
- Please keep the forms – you will need to bring them to campus to upload during a scheduled “Healthy Roster” meeting.
- All additional medical eligibility/liability forms are found and completed on the Healthy Roster website/app. We will need to give you an “invitation" to be able to access the platform to complete/upload your forms but you can go to your phone’s App Store and download the app at any time prior to meeting with us.
- Through your coach we will arrange a time to go over and have you fill out the forms on healthy roster
For those students not yet enrolled in KINES-199:
If you are not in the KNICA-199 course, you can still complete your physical, however, you will have to make sure to get added to the KNICA-199 course by contacting your coach.
