Medical Clearance Process for Student-Athletes
You are not able to practice until your physical and all the medical clearance paperwork is completed and verified in Healthy Roster.
- Enroll in the KNICA 199 out-of-season conditioning class for your specific sport.
- Print and complete this Medical History form.
- Print and fill in your name and birth date on this Physical Clearance Form.
- Schedule your physical. The physical form must be completed and signed by an MD (medical doctor), DO (Doctor of Osteopathy), FNP/NP (Nurse Practitioner) or PA-C (Physician Assistant). We are not allowed to accept a physical performed/signed by a physical therapist or chiropractor.
- Take both forms with you to your physical appointment to be completed and signed. Please ensure that the form is signed and that one of the two “Cleared for all sports without restriction” boxes is checked. The physical form will also need to be stamped by the office to verify that you were there. The Medical History form will not be signed by the physician but will just help the physician do a more thorough physical.
- Please keep both forms – you will need to bring them to campus to upload during a scheduled “Healthy Roster Meeting”.
- You will get an invitation from our sports medicine team to join Healthy Roster, our medical record and communication tool. Go to your phone’s App Store and download the Healthy Roster app at prior to the Healthy Roster Meeting. You will use the code in your invitation to create your log-in information in the app. As soon as you get your invitation to Healthy Roster, please use the code to create your login in the app. It needs to be done prior to your “Healthy Roster Meeting”.
- A time for your “Healthy Roster Meeting” with your team will be scheduled to walk you through how to upload and/or fill out the required forms on Healthy Roster. Prior to this Healthy Roster Meeting your coach will send you information on what to expect and what you need to bring with you.
For those students not yet enrolled in KINES-199:
If you are not in the KNICA-199 course, you can still complete your physical and go through the Healthy Roster Meeting. However, you will have to make sure to get added to the KNICA-199 course before you can practice.
